What MPV measures

MPV is calculated automatically as part of a complete blood count, using the same sample your lab uses to count your platelets. It reports the average volume of those platelets in femtoliters. Larger values point toward younger, more active platelets, while smaller values tend to reflect older, less reactive ones.

What high or low levels mean

A high MPV usually means your marrow is releasing younger, larger platelets, a sign of faster turnover. You may see this when platelets are being used up or destroyed and your body replaces them quickly, or alongside factors such as smoking, high blood pressure, or diabetes. A low MPV can appear with some bone marrow conditions that slow platelet production, and with certain inflammatory illnesses. MPV means little on its own, so read it together with your platelet count: a high MPV with a low platelet count suggests active replacement, while a low MPV with a low count can point toward a production problem. If your result sits well outside the range or pairs with an abnormal platelet count, follow up with your clinician.

What does a high MPV mean?

A high MPV means your platelets are larger than average, which usually reflects a batch of young, freshly made cells. On its own it is not a diagnosis, but it can prompt a closer look at your platelet count and clinical picture.

Should I worry about a low MPV?

A single low MPV is rarely a cause for alarm, especially when your platelet count is normal. Persistently low values, or a low MPV paired with a low platelet count, are worth reviewing with your clinician.

Can MPV change between tests?

Yes. MPV can drift with how long your sample sits before analysis, the anticoagulant in the tube, and normal biological variation. This is one reason you should read trends over time rather than a single value.
